How can I get started?
Call to schedule your Initial Comprehensive Exam and say you’re interested in signing up for a membership plan.
What payments are accepted?
We accept Cash, Checks or Major Credit Cards (VISA, MC, AE Discover, and of course Debit) with proper identification. We also accept Care Credit for treatment over $250, but there will a 5% reduction in your discount if you use this method.
What are my Membership start and end dates?
Your membership begins the day your payment is received and covers you for the next 12 months. Membership renewal fees are due annually on your initial sign up date. Once you have paid and are a member, the services listed are included with your annual membership and covered at 100% for the next 12 months. Treatment diagnosed is discounted by 20% and full payment is due on the day of service. Failure to pay terminates the membership agreement.
What if I’m already a patient of Dr. Brad’s, can I still sign up for a Membership Plan?
YES! If you’ve been coming in for regular care and exams within the last year, you will not need to complete the Initial Comprehensive Exam, and you are eligible to sign up for the Membership that has already been deemed appropriate for you. However, if you have not had an exam in the last year, you will need to start with the Comprehensive Exam to assess your current conditions.
Can I transfer my benefits to family members or friends?
No, each patient must have their own Membership Plan.
Can remaining benefits be carried over to the next benefit year?
No, all benefits are only good for 12 consecutive months. We are happy to schedule your preventive and periodontal visits in advance so that you will have a time reserved that works for your schedule. It is the responsibility of the patient to prearrange their visits within their 12 month membership period to maximize benefit coverage. If benefits remain at the end of membership, the member will not be entitled to a refund of any unused benefits.
What if I have to cancel a cleaning appointment?
If you call us 48 hours before your scheduled visit, then it’s no problem to reschedule your appointment to a time that is more convenient for you. However, if you give us less than the required 48-hour notice, the canceled visit will count as one of your cleaning benefits for the year. We ask for 48-hour notice so that we have time to offer available openings to other patients.
